All clients are required to provide a valid government-issued photo ID before receiving any services. Acceptable forms of ID include a driver's license, or passport. In addition, you will be required to fill out a release form at every visit, and we will attach a copy of your ID to your file to ensure accuracy and safety. All information is kept confidential.

ID

At Mad Ethel's Tattoo, we require a deposit of $50 for each appointment. This deposit is non-refundable but will be applied to the final cost of your tattoo removal treatment. Please note that we require 72-hour notice for any rescheduling. We understand that plans can change, but the deposit allows us to secure your appointment and helps us cover any costs associated with rescheduling.

Deposit

Thank you for your interest in laser tattoo removal at Mad Ethel's Tattoo! We require a non-refundable consultation fee of $50, which will be applied to the final cost if you decide to proceed with treatment. This fee covers the cost of your initial consultation with our skilled aesthetician, who will provide you with an accurate estimate and answer any questions you may have. Please note that our 72-hour notice and cancellation policies apply to consultation appointments.

Consultation Fee

We require a minimum of 72 hours notice for any cancellations or rescheduling of appointments. Deposits are non-refundable, but may be applied to a future appointment within 3 months, if cancelled with at least 72 hours notice. Please keep in mind that if an appointment is rescheduled with less than 72 hours notice or more than twice, deposits will be forfeited. At Mad Ethel's Tattoo, we understand that things come up and situations change. We have a cancellation policy in place to ensure that we can run our business and continue to provide a quality experience to our clients. Please review our policy below before booking your appointment:

 

All deposits made are non-refundable, but we are happy to work with you to reschedule your appointment if necessary. If you need to reschedule, your deposit can be kept on file for up to three months. However, if you do not reschedule in that time, your deposit will be forfeit. Please note that if an appointment is rescheduled more than twice, your deposit will be forfeited.
